PRE-ARRANGEMENT SERVICES
*Please Note: A thirty day notice is recommended for pre-arrangement. In the event that your pet passes prior to the recommended time and your form and payment has not been received your balance is due at time of completion of the cremation.

Pre-arrangments can be made in monthly installments if necessary. Partial payment is required at time of registeration.

In the event that you have completed a pre-arrangment form and paid for your services through Tender Heart Pet Memorial please inform your Vetenerian of the pre-arrangment , and let them know you have paid for your pets cremation. You may register your pet with us by completing the pre-arrangement form and mailing it to the Tender Heart Pet Memorial 210 Two Oaks Drive, Nicholasville, Ky 40356, or just stop by our facility to complete the necessary form. The price of your pre-arrangement will be honored no matter how long it is before you need our services.
